St. Charles County Ambulance District complies with the Health Insurance Portability and Accountability Act [HIPAA], and as such, is glad to provide patients with a copy of medical records associated with their ambulance trip(s). Patients also may authorize release of records to other individuals.
Please download and complete the Authorization to Release Medical Records form and submit to the District’s Custodian of Records with a copy of the patient’s photo ID (required) and Power of Attorney documents (if applicable), in person to our Headquarters address (2000 Salt River Road, St. Peters MO).
